This form should be used to update an RMR designations for a unit, or to inactivate a current RMR.  A few guiding principles should be considered BEFORE completing this form:

1) If you are no longer the RMR or Alt. RMR for a unit, you must indicate your previous unit on this from. 

2) RMRs/Alt. RMRs should be full time, benefits eligible staff and not temporary, hourly or student workers. 

3) Our office will review your submission and contact you with any questions.
